1. Application Form filled (Attached document) with one passport sized picture. Please paste it on the Application Form.
2. The passport must be submitted with the documents. All passports must have at least 6 months of validity on the day of arrival to Costa Rica. 4. Copy of all the pages of the passport.

5. Police clearance certificate (from Regional Passport Office) duly apostilled from Ministry of External Affairs and its official translation into Spanish (also duly apostilled from Ministry of External Affairs). A photocopy of both the documents, we return the originals after processing the application.
6. Round trip flight itinerary (maximum 30 days between arrival and departure of Costa Rica).
7. Proof of financial support, with sufficient bank balance (original Bank statement, 3 months). Bank account details (original document with the seal and stamp of the bank which includes the average balance, debit, and credit history of the last 3 months).
8. Salary slips for the last three months, in the case of a salaried applicant. Or business details and bank statements of the company in case of a proprietor.
9. Accommodation proof in Costa Rica. Hotel reservation. Or if the expenses of the trip are borne by a Company or by someone else, then please present also that Company’s or that person’s letter stating that they will be responsible of all the expenses regarding the accommodation. In this case, an invitation letter in Spanish from Costa Rica along with the ID proof of the person signing the letter is required
10. Income Tax Return (ITR) for the last 3 years, as applicable.
    
    IN CASE OF APPLICATION OF MINORS (under 18 years of age)
11. If the applicant is a minor and traveling alone or with only one parent, an affidavit signed by both parents and notarized by a Notary Public must be submitted. The affidavit must state that the applicant's parents authorize the applicant to travel outside India and will bear all expenses, if any. A Spanish translation of the affidavit must also be submitted.
12. Marriage certificate of parents and copy of both sides duly apostilled by the Ministry of Foreign Affairs and its official translation into Spanish duly apostilled by the Ministry of Foreign Affairs.
13. Birth certificate of the minor and copy on both sides and duly apostilled from the Ministry of Foreign Affairs, and its official translation into Spanish duly apostilled from the Ministry of Foreign Affairs.
14. The minor must attend an interview with the parents who signed the affidavit if required.
